Thanks for sharing. You motivated me to cleanup my various PDF and CSV versions of Standards Real Book. 
Here's my CSV file, including composers. Be aware that it refers to a different PDF (differences regarding page numbers). 
Column "Index" contains the page numbers as printed in the book.
